Why an Old Dog's Eyes Look Cloudy or Hazy

Most of the haze you're seeing in an old dog's eyes is a normal lens change called nuclear sclerosis, not a cataract — but the only way to be sure is to have someone look, because the two conditions are treated completely differently.

Bear was a ten-year-old golden retriever who’d always tracked a thrown tennis ball out of the air without effort. His owner brought him in because, over about four months, his eyes had gone from clear to a soft blue-gray, most noticeable in bright light or in photos. She was certain he was going blind. He wasn’t. On exam, light passed straight through both lenses, the fundus was easily visible, and he still caught the ball on the first bounce in the parking lot. What she was seeing was nuclear sclerosis — the lens hardening that comes with age — not a cataract.

That distinction is the one thing to sort out before doing anything else, and it’s why I don’t lead with home remedies or supplements. I lead with the exam.

The haze has two different causes, and they are not interchangeable

Nuclear sclerosis and cataracts can look almost identical to an owner glancing across the room, and that’s the entire problem. According to Cataracts vs. Nuclear Sclerosis: ACVO, nuclear sclerosis is a bluish-gray haze centered in the nucleus of the lens that develops as older lens fibers get compressed by new fiber growth over the dog’s lifetime — it’s a normal, expected consequence of aging, not disease. A cataract is a true opacity of the lens itself, and per the Merck Veterinary Manual, the two can and often do appear together in the same aged lens, which is part of why they get confused.

The functional difference is what matters. With nuclear sclerosis, light still passes through the lens; a veterinarian using retroillumination can see straight to the retina, and the dog’s vision is largely unaffected. With a cataract, light is blocked, vision is genuinely compromised, and depending on how far it progresses, inflammation and secondary glaucoma become real risks. One is cosmetic aging. The other is a disease process that can take a dog’s sight. Telling them apart isn’t optional — it’s the entire diagnostic question.

What I actually check for, and what I tell owners to watch

I don’t take an owner’s description of “cloudy eyes” at face value, and neither should you. The exam I do is short: retroillumination to see if I can visualize the fundus through the lens, a check of pupillary light response, and a look for any behavioral vision changes — hesitating on stairs, missing a thrown toy, bumping into furniture rearranged in a familiar room. Nuclear sclerosis, per the Learn More About the Eye Lens & Cataracts: ACVO eye lens overview, typically starts appearing by around six to eight years of age and progresses slowly and symmetrically in both eyes. A cataract can progress much faster, can be one-sided, and in dogs is most commonly tied to diabetes or a hereditary predisposition rather than to age alone.

The red flags that move me from “let’s monitor” to “let’s refer for a full ophthalmic workup” are: asymmetry between the two eyes, a white rather than blue-gray haze, redness or squinting, a sudden change over days rather than months, or any actual vision loss the owner can point to. None of those are consistent with plain nuclear sclerosis, and I don’t sit on them.

Where supplements fit, and where I stop making promises

This is the part where I get the most pushback, because owners want something to do, and there are ingredients with real biology behind them for general cellular and antioxidant support. Quercetin is a flavonoid studied for its antioxidant properties and their role in immune and cellular support. Resveratrol is an antioxidant studied for its role in supporting cellular health and vitality. Nicotinamide riboside is an NAD+ precursor studied for its support of normal cellular function. None of that is a claim that any ingredient reverses lens haze, restores vision, or treats a cataract, and I won’t write it that way, because the evidence doesn’t say that. If you look at the underlying research, the caution gets sharper, not looser: a study published in Phytomedicine found that embryonic exposure to a resveratrol analog accelerated developmental timing but shortened adult lifespan in the model organism used, and separate in vitro work in Pharmaceuticals found that quercetin and related antioxidant compounds, at the concentrations tested, didn’t even change feeding behavior in the worm model studied. That’s not a condemnation of the ingredients — it’s a reminder that in vitro and invertebrate-model data doesn’t translate directly to a dog’s eye, and nobody should be marketing it that way.

I read every label the same way I’d read a drug insert before dosing a patient: what’s the compound, what’s the dose per serving, what species was it actually tested in, and does the claim on the front of the bag match the evidence on the back. An owner survey is not a clinical trial, full stop — one company’s own March 2025 owner survey of 193 dogs reported 97.8% visible improvement after the first jar, 94% cognitive function, and 98% energy, and I cite it as exactly what it is: an observational, company-run survey, not peer-reviewed evidence of anything happening at the level of the lens. I’ve had this conversation more times than I can count, standing in an exam room with an owner who’s already decided their dog is losing its sight and just needs me to confirm it. Most of the time — not always, but most of the time — I get to tell them the opposite. I had a fourteen-year-old dachshund, overweight, mild grade-two heart murmur, whose owner had already priced out cataract surgery before the visit. Retroillumination showed a clean fundus in both eyes. The haze was nuclear sclerosis, full stop, and the dog was navigating her house and yard exactly as well as she had at eleven. I still told the owner what to watch for going forward, because nuclear sclerosis doesn’t rule out a cataract developing later on the same lens, and the two are not mutually exclusive over a dog’s remaining years.

Where this leaves you

If your old dog’s eyes have gone soft blue-gray gradually, symmetrically, over months to years, and their vision seems intact, nuclear sclerosis is the most likely explanation and it typically needs nothing more than watching. If the haze is white, one-sided, sudden, or paired with any real change in how your dog moves through a room, that’s a cataract-until-proven-otherwise, and it needs an actual exam, not a supplement trial. Antioxidant and NAD+-precursor ingredients have legitimate general cellular-support data behind them, and I don’t dismiss them, but I also won’t let a label imply they treat a lens problem when the evidence doesn’t go there.
